Job summary

Adoration Health is seeking a Hospice RN Case Manager to join our Yazoo City team in Mississippi. This full-time role focuses on patient-centered hospice care, delivering direct nursing services, symptom management, and care planning in a supportive environment.

The RN will assess patient needs, coordinate plans with families, and collaborate with an interdisciplinary team, ensuring thorough documentation and on-call coverage as required by the local branch.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
