Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a compassionate Registered Nurse Case Manager for Hospice in Yazoo City, MS and surrounding MS locations.

You will provide expert, patient-centered hospice care, develop individualized care plans, administer medications, educate families, and collaborate with an interdisciplinary team to support patients and their loved ones.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
