Job summary

Adoration Health's Hospice team in Yazoo City, MS is seeking a compassionate Registered Nurse Case Manager to provide patient-centered care and coordinate services for families during end-of-life care.

This full-time RN role emphasizes autonomous practice with team collaboration, onsite responsibilities, and rigorous documentation. A Mississippi RN license and hospice or related experience are preferred.

Qualifications

• Graduate of an accredited nursing program with active RN license in state of operation.
• Minimum 1 year nursing experience; 3+ years preferred, hospice experience preferred.
• Knowledge of hospice philosophy and care; commitment to clinical excellence.
• Must have reliable vehicle and valid driver's license and insurance.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
