Adoration Health in Yazoo City, MS, is seeking a dedicated Registered Nurse Case Manager to join our hospice team. You will provide patient-centered care at a critical time, assess needs, and develop individualized care plans.

The role offers independent practice with team support, on-call rotation, and opportunities for professional growth. A valid RN license and hospice experience are preferred, with a focus on compassionate, high-quality care.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must have a valid driver's license and insured vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
