Job summary

Stillwater Hospice, LLC is seeking an experienced Hospice RN Case Manager in Kalispell, Montana. The RN will plan, organize, and direct hospice care, coordinating with the interdisciplinary team to meet patient and family needs in home settings and communities.

The role emphasizes nursing expertise, community health education, and collaboration with a dynamic team. On-call duties and ongoing professional development are included.

Qualifications

• Graduate of an accredited school of nursing.
• One to two years of recent acute care experience in an institutional setting.
• One to two years of recent experience in home care.
• Current licensure in State.
• Licensed driver with insured vehicle.

Responsibilities

• Complete initial and ongoing patient/family assessments to determine hospice needs.
• Provide professional nursing care per nursing process and act within Nurse Practice Act.
• Develop and revise care plans with patient/family involvement.
• Administer medications and treatments as prescribed.
• Coordinate patient care as Case Manager withClinical Supervisor.
• Communicate with physicians and care team; educate patients and families.
• Document care and outcomes in medical records.
• Participate in interdisciplinary team meetings and quality improvement activities.
