Job summary

Stillwater Hospice is seeking a Registered Nurse Case Manager in Kalispell, MT. This role plans, organizes, and directs hospice care with emphasis on community health education and patient-family support.

The RN coordinates care, conducts assessments, develops plans of care, administers treatments, and collaborates with an interdisciplinary team to ensure high-quality end-of-life care. On-call duties and ongoing professional development are also part of the position.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of recent acute care experience; 1–2 years in home care preferred.
• Current licensure in State.
• Must have a valid driver’s license and reliable vehicle.
• Bachelor’s degree preferred; hospice care experience valued.
• Supervisory experience not required; may oversee hospice aides.

Responsibilities

• Complete initial and ongoing patient/family assessments to determine hospice needs.
• Develop and revise the plan of care based on nursing diagnosis and patient needs.
• Administer medications and treatments as prescribed; provide nursing care per physician plan.
• Counsel patients and families on nursing and related care needs; provide education.
• Coordinate care across the interdisciplinary team; participate in meetings.
• Ensure safe, high-quality care and maintain accurate clinical notes.
