Job summary

Stillwater Hospice is seeking a Hospice RN Registered Nurse Case Manager in Helena, MT. The RN directs hospice care, performs comprehensive assessments, and builds a plan of care with input from patients and families.

Responsibilities include initiating and revising plans, managing medications and therapies, educating families, and coordinating with the interdisciplinary team for optimal end-of-life care.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years acute care in institutional settings and 1–2 years home care.
• Current RN licensure in the State.
• Bachelor's degree; hospice care experience preferred.

Responsibilities

• Conduct initial and ongoing patient/family assessments to determine hospice needs.
• Develop and revise care plans with nursing diagnoses and goals.
• Administer medications and treatments as prescribed and educate patients/families.
• Coordinate care as Case Manager with the interdisciplinary team.
• Provide health teaching and ensure safe, quality home-based care.
