Embrace Hospice in Kerrville, TX is seeking a passionate Hospice RN Case Manager to provide 1:1 patient care within a supportive team. Schedule is M-F 8-5, with flexible options and opportunities for advancement as part of a growing company.

You will collaborate with an interdisciplinary team to deliver compassionate, patient-centered care in home settings. Responsibilities include assessing patient needs, creating care plans, administering therapies, managing pain and symptoms, and educating

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications
