Job summary

Embrace Hospice is seeking a Registered Nurse Case Manager to join our Kerrville, TX team. The role focuses on patient-centered hospice care, creating individualized care plans, and collaborating with an interdisciplinary team to support patients and families throughout the care journey.

The RN will provide direct nursing care, administer medications, manage symptoms, and ensure accurate documentation while participating in on-call rotations.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
