Job summary

KWZ Solutions is seeking a compassionate Hospice Registered Nurse (RN) Case Manager in Ames, IA. This field-based, full-time role focuses on delivering end-of-life care, coordinating across a multidisciplinary team, and advocating for patient and family goals.

You will manage caseloads, conduct assessments, and ensure plans of care align with comfort and dignity. Travel within ~60 miles, with an on-call rotation and a Monday–Friday schedule.

Qualifications

• Active RN license in the State of Michigan.
• Experience in hospice, home health, palliative care, or related nursing.
• Proven patient assessment and case management skills.

Responsibilities

• Provide comprehensive nursing care to hospice patients in home and community settings.
• Complete initial and ongoing patient assessments and update care plans as clinical needs evolve.
• Develop and implement individualized Hospice Plans of Care with physicians and interdisciplinary team.
