Job summary

Resource 1 Home Staffing in West Des Moines, IA seeks a Hospice Registered Nurse (RN) Case Manager to provide comprehensive hospice care. This full-time role (40 hours weekly) involves home-based patient care, on-call duties, and coordination with an interdisciplinary team to meet patient and family needs.

The RN will assess patients/families, develop and adjust care plans, administer medications as prescribed, educate families, and ensure quality end-of-life care in home and community settings.

Qualifications

• 1-2 years of recent acute care experience in an institutional setting (required).
• 1-2 years of recent home care experience (preferred).
• Graduation from an accredited school of nursing.
• Understands hospice philosophy and end-of-life care.
• Empathy for patients and families; calm in home settings.

Responsibilities

• Completes initial and ongoing patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises the plan of care with patient/family involvement.
• Initiates and revises care plans as patient needs change.
• Coordinates patient care and participates in on-call duties.
