WesleyLife At Home Hospice in Urbandale, IA is seeking a dedicated Hospice RN Case Manager to coordinate patient care in home settings and support families through the hospice journey. As an RN Case Manager you will travel to patients, assess needs, manage symptoms, develop care plans with the inter-disciplinary team, educate caregivers, and provide compassionate presence during difficult times.

Qualifications

• Must have current vehicle insurance.
• Must hold an Iowa RN license.
• Minimum two years of full-time nursing experience; hospice experience preferred.

Responsibilities

• Travel to patients to conduct nursing assessments and monitor symptoms.
• Manage pain and other symptoms using hospice practices.
• Coordinate medications, treatments and supplies for patient needs.
• Prepare and update individualized care plans with the interdisciplinary team.
• Educate patients and families about the hospice journey and care expectations.
• Teach family caregivers how to provide safe, comfort-focused care.
