WesleyLife Hospice seeks a Registered Nurse Case Manager to provide in‑home hospice care, travel to patient homes, and collaborate with an interdisciplinary team to monitor symptoms and comfort. The role emphasizes compassionate, patient‑ and family‑centered care across diverse settings.

The ideal candidate holds an Iowa RN license, has at least two years of full‑time nursing experience (one year hospice preferred), and possesses a valid Iowa driver’s license with vehicle insurance.

Qualifications

• Valid, unrestricted Iowa driver’s license.
• Current vehicle insurance.
• Current license as a registered nurse in the state of Iowa.
• Two years of full-time nursing experience required, with one year of hospice experience preferred.

Responsibilities

• Travel to each patient to conduct thorough nursing assessments to monitor symptoms, comfort levels, and overall well‑being.
• Manage pain and other symptoms using evidence‑based hospice practices.
• Coordinate medications, treatments, and supplies to support patient needs.
• Prepare and update individualized care plans in collaboration with the interdisciplinary team.
• Provide education on what to expect throughout the hospice journey.
• Teach family caregivers how to provide safe, comfort‑focused care.
• Offer a listening ear and calm presence during moments of uncertainty.

Education

RN license (Iowa)
