Job summary

Iowa City Hospice is seeking a Hospice Nurse Case Manager (RN) to coordinate patient-centered care in homes and care facilities. You will work independently and with a seasoned interdisciplinary team to ensure comfort and dignity at end of life.

Responsibilities include creating plans of care, administering meds, and guiding families through caregiving decisions with compassionate, clinical expertise. Bachelor's degree or RN/BSN/ARNP eligibility preferred.

Qualifications

• RN/BSN/ARNP license and current Iowa nursing license required or expected.
• Two years of home health or hospice experience preferred; clinical knowledge in end-of-life care desirable.
• Must have a car, valid driver's license, and required insurance per policies.
• Strong clinical knowledge, critical thinking, and evidence-based practice.

Responsibilities

• Develop and implement individualized care plans based on patient goals and preferences.
• Deliver comprehensive nursing care in home and facility settings.
• Coordinate medical, emotional, psychosocial, and spiritual support with the interdisciplinary team.
• Perform ongoing assessments and adjust care plans as patient needs evolve.
• Administer medications and manage complex symptoms for comfort and quality of life.
• Educate patients and families on disease progression and caregiving strategies.
• Act as advocate and resource for patients and families navigating end-of-life decisions.
• Collaborate with physicians, APPs, and team members; participate in quality initiatives.
