Job summary

Gentiva is seeking a Hospice RN / RN Case Manager for PRN roles with flexible days and hours. You will be the primary contact for patients and families, delivering skilled nursing care in patients' homes or other residences, and coordinating with an interdisciplinary team to create and update plans of care.

CPR and an active RN license are required. You will perform ongoing assessments, educate caregivers, and document care accurately while adhering to regulatory standards and infection control

Qualifications

• Current RN license in the state of employment.
• CPR certification.
• Current automobile insurance and valid driver’s license.
• Hospice or palliative care experience preferred.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care based on each patient’s Plan of Care in home settings.
• Perform initial and ongoing assessments to monitor patient condition.
• Collaborate with an interdisciplinary team to refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers with practical training and reassurance.
• Communicate updates in patient condition to the care team for timely interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure compassionate care.
• Explain hospice services, medications, symptom management, and end-of-life processes.
