Bristol Hospice is seeking a Registered Nurse Case Manager to accompany patients and families on their hospice journey. You will assess needs, develop care plans, and coordinate compassionate, holistic support across medical, emotional, and spiritual dimensions.

The role emphasizes strong communication, diligent documentation, and teamwork while delivering dignified end-of-life care in a mission-driven environment.

Qualifications

• RN license in good standing in the state you will work
• CPR certification current
• Valid driver’s license and reliable transportation
• Strong observational and communication skills both written and verbal
• Team-oriented attitude and flexibility to support additional duties

Responsibilities

• Develop and implement individualized care plans based on patient history and current needs
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Monitor and document vital signs and changes in patient condition with accuracy and timeliness
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and families on care techniques and symptom management
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• Maintain detailed clinical notes reflecting patient progress and quality of care
