Job summary

Bristol Hospice is seeking RN Case Managers to provide compassionate, whole-person care and advocate for patients and families. This role supports a family-centered hospice journey and collaborates with an interdisciplinary team to ensure comfort, dignity, and quality of life.

You will evaluate needs, develop individualized care plans, monitor vitals, educate families, and document patient progress throughout the care continuum.

Qualifications

• Valid Registered Nurse license in good standing in the state you will work in.
• CPR certification current.
• Strong observational and communication skills—written and verbal.
• Flexible, team-oriented attitude; willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
