Job summary

Bristol Hospice is seeking an RN Case Manager who will provide compassionate, whole-person care and act as an advocate for patients and families. Join a team focused on dignity, respect, and a mission-driven culture as you coordinate care across disciplines.

You will develop individualized plans, monitor vital signs, and educate families on symptom management, while delivering comfort and support in every interaction.

Qualifications

• A valid Registered Nurse license in good standing in the state you will work in.
• CPR certification (current).
• Strong observational and communication skills—both written and verbal.
• A valid driver’s license and reliable transportation.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes reflecting patient progress and quality of care.
