Job summary

Bristol Hospice is seeking a devoted Registered Nurse Case Manager to guide patients and families through the hospice journey with compassionate, whole-person care. You will conduct assessments, develop individualized care plans, and collaborate with interdisciplinary teams to provide dignified end-of-life care in the Portland area.

The role emphasizes advocacy, education, and meticulous documentation, with a culture rooted in integrity, trust, excellence, accountability, and mutual respect.

Qualifications

• Active RN license in good standing for the state you will work in.
• Experience in hospice care is preferred.
• Ability to develop and implement individualized care plans.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Provide one-on-one support to patients and families ensuring comfort and understanding.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor vital signs, document findings, and report changes promptly.
• Administer physician-ordered treatments with empathy and accuracy.
• Educate patients and families on care techniques and symptom management.
• Maintain detailed clinical notes reflecting patient progress and quality of care.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
