Bristol Hospice is seeking an RN Case Manager to provide compassionate, whole-person care and advocate for patients and families on the hospice journey. You will develop individualized care plans, monitor vitals, and educate families while coordinating with interdisciplinary teams to ensure comfort, dignity, and high-quality care.

Qualifications

• Valid Registered Nurse license in the state of work.
• CPR certification current.
• Excellent observational and communication skills.

Responsibilities

• Develop and implement individualized care plans for patients.
• Provide one-on-one support to patients and families, ensuring comfort and dignity.
• Document vital signs and clinical notes, monitor changes and respond appropriately.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams for comprehensive care delivery.
