Job summary

Bristol Hospice is seeking dedicated RN Case Managers to support patients and families through the hospice journey with compassionate, person-centered care. You will collaborate with interdisciplinary teams to ensure comfort, dignity, and quality of life, while documenting progress and educating families on management of symptoms.

Candidate must hold an active RN license in the applicable state, CPR certification, and have reliable transportation.

Qualifications

• Valid RN license in the state you work in.
• CPR certification (current) expected.
• Strong communication and compassionate patient care.
• Reliable transportation and valid driver’s license.

Responsibilities

• Develop and implement individualized care plans based on medical history and current needs.
• Deliver one-on-one support to patients and families, ensuring comfort and dignity.
• Monitor vital signs and administer physician-ordered medications and treatments.
• Educate patients and families on care techniques and symptom management.
• Document patient progress and care notes; coordinate with interdisciplinary teams.
