Bristol Hospice is seeking an experienced RN Case Manager in Atlanta, GA to provide compassionate, whole-person care and advocacy for patients and families.| You will develop individualized care plans, monitor clinical status, and collaborate with an interdisciplinary team to ensure comfort and dignity at every stage of the hospice journey.

Qualifications

Valid Registered Nurse license in good standing in the state of practice.
Strong observational and written/verbal communication abilities.
Ability to collaborate with interdisciplinary teams to plan care.

Responsibilities

Develop and implement individualized care plans based on patient history.
Provide compassionate, one-on-one support to patients and families.
Assess physical, emotional, and spiritual needs and document findings.
Monitor vital signs and administer physician-ordered treatments with care.
Educate families on care techniques and symptom management.
Maintain detailed clinical notes and coordinate with care teams.
