Job summary

Bristol Hospice in Watkinsville, GA is seeking an RN Case Manager to advocate for patients and guide families through the hospice journey with empathy and professionalism. You will be responsible for developing personalized care plans and coordinating interdisciplinary care to ensure comfort and dignity.

You will monitor patient status, educate families, document thoroughly, and collaborate with clinicians to deliver holistic end-of-life care in a supportive setting.

Qualifications

• Must hold a valid RN license in good standing in the state you will work in.
• Valid driver’s license and reliable transportation.
• CPR certification (current).

Responsibilities

• Develop and implement individualized care plans based on patient history and current condition.
• Deliver one-on-one support to patients and families, ensuring comfort and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs (temperature, pulse, respiration, blood pressure).
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.
• Track and communicate changes in patient condition for timely action.
• Maintain detailed clinical notes reflecting patient progress and quality of care.
