Job summary

Qualifications

• RN license in the state of employment.
• CPR certification must be current and active.
• Hospice or palliative care knowledge is beneficial.

Responsibilities

• Perform comprehensive patient assessments and reassessments and educate patients, families and caregivers.
• Deliver nursing interventions and symptom management per goals of care.
• Update individualized plans of care including visit frequency to meet needs.
• Coordinate care across interdisciplinary teams and consult with physicians.
• Educate patients and caregivers on disease, self-care, end-of-life care and medications.
• Document timely clinical records and maintain care plans.
