Gentiva Hospice is seeking a compassionate RN Case Manager to provide in-home hospice and palliative care across Texas. You will perform initial and ongoing assessments, deliver skilled nursing care per the Plan of Care, and collaborate with physicians, social workers, and other team members to tailor care plans.

You will educate families, support bereavement needs, and ensure regulatory compliance. As the primary point of contact, you will coordinate care, document findings, and help families

Formación

• Current RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Minimum 1 year as an RN in acute care, hospice, home health, or related settings.

Responsabilidades

• Perform comprehensive patient assessments and ongoing reassessments, including education for patients and caregivers.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Coordinate care with an interdisciplinary team to refine care plans.
• Document care activities, ensure infection control and regulatory compliance.
• Support families with education, bereavement guidance, and transitions in care.
