Job summary

Gentiva in Texas seeks a dedicated Hospice RN / RN Case Manager to lead compassionate end-of-life care in patients' homes and community settings. You will assess needs, implement plans of care, and collaborate with a multidisciplinary team.

You will educate families, coordinate with physicians and aides, document care, participate in on-call coverage, and support quality improvement to ensure safe, dignified care. Strong communication and clinical assessment are essential.

Qualifications

• Registered Nurse licensed to practice in the state of employment.
• Minimum of one year RN experience in acute care, oncology, long-term care, hospice or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across Interdisciplinary Teams with physicians and other providers.
• Educate patients and caregivers on disease, self-care, and end-of-life options.
• Communicate changes in condition to the care team and document findings.
• Support transitions between levels of care and handle on-call duties as assigned.
