Gentiva Hospice is hiring a Hospice RN / RN Case Manager to lead patient care in the home setting. You will assess patients, document care, and coordinate a plan with an interdisciplinary team to ensure compassionate, personalized end-of-life services.

You will educate patients and families, monitor symptoms, and adjust plans as needs change, while maintaining compliance with regulatory standards and quality initiatives.

Qualifications

• Minimum of 1 year RN experience in hospice, home health, or related settings.
• Hospice or palliative care experience preferred.

Responsabilités

• Perform comprehensive patient assessments and reassessments in person or via telecommunication.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized care plans with visit frequency.
• Coordinate care with physicians and IDG members.
