Gentiva Hospice is seeking a dedicated RN Case Manager to deliver high-quality hospice care in patients’ homes. You will assess and manage symptoms, develop care plans, and coordinate with an interdisciplinary team to support patients and families during end‑of‑life care.

The role emphasizes compassionate communication, detailed documentation, and education for patients and caregivers, with on‑call responsibilities and a strong focus on regulatory compliance and quality improvement.

Qualifications

• RN license in good standing.
• Minimum 1 year RN experience (hospice/home health/acute care).
• CPR certification.
• Current automobile insurance and valid driver’s license.
• Ability to work in patients' homes and coordinate with IDG.

Responsabilités

• Perform comprehensive patient assessments and reassessments, educating patients, families and caregivers as appropriate.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams and consult with physicians.
• Educate patients and caregivers about disease processes, end-of-life care and medications.
