Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to provide in-home hospice care across McMinnville and surrounding areas. You will assess, treat, document, and educate patients and families within an individualized care plan.

Join a compassionate team delivering holistic end‑of‑life care, collaborating with physicians, social workers, chaplains, and other professionals to ensure comfort, dignity, and support. On-call rotation included.

Qualifications

• Current license to practice nursing in the state of employment.
• CPR Certification.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments with education for patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management per care goals.
• Develop and update individualized plans of care and determine visit frequency.
• Coordinate care across IDG with physicians and other providers.
• Educate patients and caregivers on disease, end-of-life care, medications and nutrition.
• Communicate changes in condition to the care team for timely interventions.
• Document timely clinical records and maintain infection control and compliance.
• Support scheduling and quality improvement initiatives to ensure compassionate care.
