Job summary

Gentiva in McMinnville, TN is seeking a Home Hospice RN / RN Case Manager to lead patient care in the home setting, coordinating with families and the care team to deliver compassionate, skilled nursing services aligned with each patient's Plan of Care.

This full-time weekday role includes initial assessments, ongoing symptom management, education for caregivers, and collaboration with physicians, LPNs, CNAs, social workers, and chaplains to support end-of-life care.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.
• Deliver and document skilled nursing care based on each patient's Plan of Care in their home or other residence.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with an interdisciplinary team to build and refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones, including bereavement guidance.
• Teach caregivers how to care for their loved one safely and confidently.
• Communicate updates in patient condition to the care team for timely interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure compassionate, efficient care.
• Explain topics such as medication administration, hospice philosophy, symptom management, and end-of-life processes.
