Job summary

Optimal Hospice is seeking a Registered Nurse Case Manager to provide comprehensive in-home and patient-care services. You will perform assessments, develop and adjust care plans, administer medications, and coordinate with physicians and the interdisciplinary team to support patients and families.

This role emphasizes compassionate, patient-centered care, adherence to nursing standards, and efficient coordination of resources. A valid Colorado RN license and hospice experience are preferred.

Qualifications

• RN licensure in Colorado.
• Experience in hospice or palliative care preferred.
• Strong assessment and care planning skills.

Responsibilities

• Performs initial and ongoing patient assessments and histories.
• Provides professional nursing care following nursing process.
• Develops and revises care plans with patient and family involvement.
• Administers medications and treatments as prescribed.
• Coordinates care with physicians and interdisciplinary team.
• Educates patient and family on self-care and plan of care.
• Ensures safety and continuity of care across settings.
• Attend interdisciplinary group meetings.
