Job summary

Gentiva Hospice is seeking an RN to provide compassionate hospice care in Terre Haute, Indiana. You will lead patient assessments, deliver skilled nursing, and coordinate with an interdisciplinary team to ensure tailored, high-quality plans of care for patients in home settings and facilities.

Ideal candidates hold an Indiana RN license and have experience in hospice or palliative care. Strong documentation, teaching, and communication skills are essential for family support and care continuity.

Qualifications

• RN license required in Indiana
• Hospice or palliative care experience preferred

Responsibilities

• Lead patients and families through hospice journey with compassionate care.
• Provide skilled nursing in patients' homes or care settings per Plan of Care.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with physicians, LPNs, CNAs, social workers, chaplains, and others.
• Ensure accurate clinical documentation and infection control.
• Educate caregivers and provide bereavement guidance.
