Job summary

Gentiva, Heartland Hospice part of a national hospice and palliative care provider, is seeking a Hospice RN to lead patient and family care in home settings. You will deliver skilled nursing, assess needs, and collaborate with doctors, social workers, and chaplains to create personalized plans.

The role emphasizes compassionate care, caregiver education, and thorough documentation, with on-call duties and adherence to regulatory standards.

Qualifications

• RN required with license to practice nursing in the state of employment.
• Hospice or palliative care experience preferred
• CPR certification required

Responsibilities

• Be the primary contact for patients and families guiding hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home or other settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDG to build personalized care plans.
• Educate caregivers and provide bereavement guidance.
• Maintain accurate clinical documentation and infection control compliance.
• Support continuity of care across home health, palliative, and hospice referrals.
