Gentiva Hospice is seeking a compassionate RN Case Manager to lead in-home nursing care and coordinate comprehensive end-of-life services. You will assess patients, implement plans of care, and collaborate with physicians, social workers, chaplains, and other care team members.

You will educate families, monitor symptoms, document care, and ensure regulatory compliance while supporting patients through the hospice journey and bereavement guidance.

Competenze

• Active RN license in the state of employment.
• Minimum of 1 year experience in hospice, home health, oncology, or related field preferred.
• Excellent assessment, communication, and documentation skills.

Mansioni

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver and document skilled nursing interventions per Plan of Care.
• Coordinate with IDG to develop and update plans of care.
• Educate patients and caregivers on disease process, medications, and end-of-life care.
• Communicate changes in patient condition to the care team for timely interventions.
• Maintain accurate clinical records and adhere to infection control standards.
• Support scheduling and quality improvement initiatives.
