Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to provide compassionate, in-home hospice and palliative care. You will assess, plan, and deliver nursing care, coordinating with an interdisciplinary team to ensure patient comfort and dignity.

Responsibilities include education for patients and families, symptom management, and thorough documentation, with on-call support as needed. Must hold an active RN license and CPR certification.

Qualifications

• Currently licensed to practice as an RN in the state of employment.
• Minimum of 1 year RN experience; hospice or palliative care preferred.
• CPR certification required.
• Valid driver's license and insurance required.
• Experience with interdisciplinary teams is essential.

Responsibilities

• Perform comprehensive patient assessments and reassessments and educate patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate on disease, end-of-life care, medications, nutrition, and self-care.
• Document clinical records timely and maintain infection control and compliance standards.
• Participate in on-call and patient support activities as applicable.
