Hospice of the West is seeking a dedicated Registered Nurse Case Manager (RNCM) for a full-time role serving the Peoria and Surprise areas. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through their hospice journey with compassion and clinical excellence.

Competenze

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Hospice of the West is seeking a Full-Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday 8a - 5p

Territory: Peoria / Surprise

In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.
