Job summary

Hearth Hospice is seeking a dedicated Registered Nurse Case Manager (RNCM) to join our compassionate nursing team in the community. This full-time role involves hands-on hospice nursing, care coordination, and guiding patients and families through the journey with compassion and clinical excellence.

The position offers a Monday–Friday schedule (8am–4:30pm, etc.) and a comprehensive benefits package designed to support professional growth and a balanced life.

Qualifications

• Hold an active, unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience is a plus.
• Proficient typing and EMR software experience required.
• CPR certification (where state requires).
• Valid driver's license, insurance, and reliable transportation.

Responsibilities

• Respond to clinical referrals with compassionate, informed guidance.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Deliver direct, hands-on hospice nursing care and coordinate services.
• Provide nursing care rooted in proven practices to ease discomfort.
• Educate patients and families to participate in personalized care plans.
• Share in on-call duties to ensure care is available when needed.
