Job summary

Frontier Hospice in Edmond, OK is seeking a full-time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team delivering end-of-life care across the community.

The role involves hands-on nursing care, care coordination, and guiding patients and families through the hospice journey, with a schedule Monday–Friday, 8 am–4:30 pm. Edmond territory and a supportive benefits package are provided.

Qualifications

• Hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
• Proficient with computers and EMR software.
• CPR certification if required by state.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Empower Through Engagement: respond to referrals with care and expertise.
• Collaborative Care Champion: work with Clinical Director to assess eligibility for hospice services.
• Nurturing Direct Care: provide hands-on hospice nursing care and coordinate services.
• Nursing with Heart: deliver care rooted in nursing principles to ease pain and comfort patients.
• Educator and Advocate: educate patients and families for personalized care plans.
• Call of Compassion: participate in on-call duties to be there when it matters most.
