Job summary

Hospice of North Carolina is seeking a Full Time Hospice RN Case Manager (RNCM) to join our nursing team providing compassionate end-of-life care throughout the community. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.

As an RN Case Manager, you will collaborate with our Clinical Director, assess eligibility for services, and ensure patient comfort and quality of life.

Qualifications

• Hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.
• CPR certification current if required by state.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to clinical referrals with compassionate, expert nursing care.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Deliver Direct Care as an RN Case Manager, prioritizing comfort and quality of life.
• Provide nursing care rooted in established best practices.
• Educate and advocate for patients and families to participate in care plans.
• Distribute call duties as part of the team.
