Heartland Hospice, part of Gentiva, seeks a dedicated Hospice RN / RN Case Manager to lead patient and family care at home or in care settings. You will perform comprehensive assessments, implement skilled nursing interventions, and coordinate with physicians, social workers, and other team members to deliver compassionate end-of-life care.

The role emphasizes education for caregivers, documentation, and adherence to regulatory standards, with on-going involvement in quality and care

Qualificações

• RN licensure in state of employment.
• CPR certification required.

Responsabilidades

• Lead patient and family through hospice journey with primary point of contact.
• Deliver and document skilled nursing care per Plan of Care in home or other settings.
• Perform initial and ongoing patient assessments and adjust care as needed.
• Collaborate with IDG to refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers on safe patient care and problem-solving.
• Communicate patient condition updates to care team for timely interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling to ensure compassionate, efficient care.
• Assist patients/families with understanding hospice services, symptom management, and end-of-life processes.
