Job summary

Continuum Hospice is seeking a dedicated Registered Nurse Case Manager (RNCM) to join our nursing team providing end-of-life care in the Warwick/Coventry area. This full-time role involves hands-on hospice nursing, care coordination, and guiding patients and families through every step of their journey.

The candidate should hold an active RN license, have at least 1 year of acute care experience, and be comfortable with EMR systems. Hospice experience is a plus; CPR may be required.

Qualifications

• Active RN license in good standing.
• Minimum 1 year RN experience in medical-surgical/acute care; Hospice experience a plus.
• Proficient with computers and electronic medical records; CPR certification if required by state.

Responsibilities

• Respond to clinical referrals with care and expertise, addressing inquiries promptly.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct nursing care and coordinate services to maintain comfort and quality of life.
• Deliver care with compassion, applying proven nursing principles and techniques.
• Educate patients and families to participate in personalized care plans aligned with goals.
• Share in on-call duties to ensure availability during critical moments.
