Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager in Mishawaka, IN to serve patients in home, assisted living, and other settings. You will lead patient and family education, perform initial and ongoing assessments, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to deliver personalized end-of-life care.

The role emphasizes clinical excellence, patient-centered communication, and adherence to regulatory standards.

Qualifications

• Active RN license in state of employment.
• Hospice or palliative care experience preferred.
• Strong communication and documentation skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver and document skilled nursing care per Plan of Care.
• Coordinate care across interdisciplinary teams to develop and refine care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Maintain timely clinical records and ensure infection control compliance.
• Participate in on-call and patient support activities as applicable.
