The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Mishawaka, IN. You will coordinate patient care, manage individualized plans of care, and collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort and dignity at end-of-life.

Qualifications

• Active RN license
• Hospice nursing experience preferred
• Experience with Homecare Homebase (HCHB) preferred
• Strong assessment, communication, and care coordination skills

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care.
• Educate patients, families, and caregivers on symptom management and end-of-life care.
