Bristol Hospice is seeking dedicated RN Case Managers to guide patients and families through hospice care with compassion and advocacy. You will develop personalized care plans, monitor health needs, educate families, and collaborate with a multidisciplinary team to ensure comfort and dignity on the journey.

Qualifications

• A valid Registered Nurse license in good standing in the state you will work in.
• CPR certification (current).
• Strong observational and communication skills—both written and verbal.
• Flexible, team-oriented attitude and willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver attentive, one-on-one support to patients and families, ensuring comfort and dignity.
• Conduct thorough assessments of physical, emotional, and spiritual needs.
• Monitor vital signs and administer physician-ordered medications.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
