Job summary

Bristol Hospice is actively seeking Registered Nurses to join our team as RN Case Managers. You’ll advocate for patients and families, build individualized care plans, and partner with the broader team to ensure comfort and dignity across the hospice journey.

The role emphasizes compassionate, whole-person care, clinical documentation, and education for families while providing support throughout the care continuum in a home- or facility-based setting.

Qualifications

• Active RN license in good standing in the state of work.
• Current CPR certification and clinical competency.
• Ability to develop individualized care plans.
• Strong communication with patients, families, and team.
• Documentation of vital signs, assessments, and progress.
• Willingness to travel as needed for patient visits.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one support ensuring comfort and dignity.
• Conduct thorough assessments of physical, emotional, spiritual needs.
• Monitor and document vital signs and patient condition changes.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
