Job summary

Saint Mary's Hospice, part of the Optum family, seeks a Registered Nurse Case Manager to lead the Plan of Care for hospice patients. You will evaluate performance, collaborate with the IDG, and implement strategies that support physical, psychosocial, emotional, and spiritual well-being in patients and families facing end-of-life care.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience preferred.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies the patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family responses.
• Collaborates with the patient/family, attending physician and IDG for daily care.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes and adapts care to diverse home settings.
