Horizon Hospice of Colorado is seeking an experienced Registered Nurse Case Manager to provide compassionate hospice care across the Denver Metro area. This 0.8 FTE role with growth to 1.0 FTE involves coordinating care as the primary clinical contact for a patient group and collaborating with the interdisciplinary team.

The RNCM will perform assessments, manage symptoms, educate families, and ensure documentation in the EMR, with opportunities to grow caseload as census increases.

Qualifikationen

• Current, unrestricted RN license in Colorado.
• Minimum of one year hospice nursing experience.
• Strong clinical assessment and symptom-management skills.
• Ability to work independently in community settings while collaborating with an IDT.
• Excellent communication, organization, and time-management abilities.

Aufgaben

• Perform comprehensive nursing assessments and ongoing reassessments.
• Develop and update individualized hospice plans of care with the IDT.
• Provide skilled nursing visits in private homes, assisted living, and SNFs.
• Manage pain and symptoms; administer treatments as ordered.
• Educate patients and caregivers on medications, progression, and end-of-life care.
• Coordinate medications, equipment, and services to support the plan of care.
