Qualifications

• Must have a current RN license in the state of Colorado.
• Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
• Strong assessment and critical thinking skills to manage complex patient needs.

• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Conduct comprehensive assessments to develop individualized care plans.
• Coordinate and oversee patient care services with a multidisciplinary team.
• Educate and support patients and families regarding end-of-life care options.
• Monitor and evaluate patient progress, adjusting care plans as necessary.
• Ensure compliance with regulatory and quality standards in hospice care.
• Document patient care activities accurately in electronic health records.
• Participate in team meetings for continuous improvement initiatives.
• Act as a liaison between patients, families, and healthcare providers.
