Job summary

Horizon Hospice of Colorado is seeking a skilled Registered Nurse Case Manager to coordinate and deliver hospice care across the Denver Metro area. You will conduct comprehensive nursing assessments, lead an interdisciplinary team, and provide direct patient visits in homes and care settings.

This full-time role starts at 0.8 FTE (32 hours per week) with growth to 1.0 FTE (40 hours). Expect a typical caseload of 12-15 patients and travel within the service area, with mileage reimbursement

Qualifications

Registered Nurse Case Manager (RNCM)

Position Summary

Horizon Hospice of Colorado is seeking an experienced Registered Nurse Case Manager (RNCM) to provide compassionate, high-quality hospice care to patients and families throughout the Denver Metro area.

The RN Case Manager serves as the primary clinical coordinator for an assigned group of hospice patients and works closely with the interdisciplinary team to ensure care is individualized, well-coordinated, and focused on comfort, dignity, and quality of life.

This position includes direct patient care, symptom management, family education, care coordination, interdisciplinary collaboration, and clinical documentation.

Schedule & FTE Structure

This is a full-time position initially structured at 0.8 FTE (32 hours per week), with the opportunity to increase to 1.0 FTE (40 hours per week) as hospice census and caseload grow.

RN Case Manager caseload expectations are based on patient census, acuity, geography, and individual care needs.

Typical full RNCM caseload: 12-15 patients, depending on patient complexity and travel requirements.

Our goal is to increase this position to 1.0 FTE as patient census supports a full RN Case Manager caseload.

Essential Responsibilities

Patient Care & Clinical Management

• Complete comprehensive nursing assessments and ongoing reassessments of patient needs.

• Develop, implement, and update individualized hospice plans of care in collaboration with the interdisciplinary team.

• Provide skilled nursing visits in private homes, assisted living communities, skilled nursing facilities, and other care settings.

• Assess and manage pain, symptoms, functional decline, and changes in patient condition.

• Administer medications, treatments, and interventions as ordered.

• Educate patients and caregivers regarding medications, symptom management, disease progression, end-of-life changes, and when to contact hospice.

• Provide support and clinical guidance during changes in condition and symptom crises.

• Perform pronouncement of death and post-mortem care when appropriate and in accordance with hospice policy.

• Complete timely and accurate documentation in the electronic medical record.

Care Coordination & Interdisciplinary Collaboration

• Serve as the primary clinical coordinator for assigned hospice patients.

• Collaborate with physicians, social workers, chaplains, hospice aides, therapists, volunteers, and other members of the interdisciplinary team.

• Participate in interdisciplinary team (IDT) meetings and contribute meaningfully to patient care planning and review.

• Communicate changes in patient status, goals, symptoms, and care needs to appropriate team members.

• Coordinate medications, durable medical equipment, supplies, and additional services needed to support the patient plan of care.

• Facilitate continuity of care when patients transition between care settings.

Patient & Family Support

• Provide education, guidance, and emotional support to patients and families throughout the hospice journey.

• Help patients and caregivers understand hospice services, the plan of care, and what to expect as illness progresses.

• Support families during the active dying process and end-of-life transition.

• Promote patient dignity, comfort, autonomy, and quality of life.

Compliance & Documentation

• Maintain clinical documentation consistent with hospice regulatory requirements, organizational policies, and standards of care.

• Complete assessments, care plans, visit documentation, orders, and care coordination notes accurately and within required timeframes.

• Support compliance with Medicare Hospice Conditions of Participation and applicable state requirements.

• Participate in quality improvement initiatives, chart audits, education, and clinical competency activities as requested.

• Maintain patient confidentiality and comply with HIPAA requirements.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the State of Colorado.

• Minimum of one year of hospice nursing experience required.

• Strong clinical assessment, critical thinking, and symptom-management skills.

• Experience caring for patients with advanced illness and end-of-life needs.

• Ability to work independently in community-based settings while collaborating effectively with an interdisciplinary team.

• Strong communication, organization, documentation, and time-management skills.

• Comfortable providing patient and caregiver education in emotionally sensitive situations.

• Valid driver's license, reliable transportation, and ability to travel throughout the service area.

• Current CPR certification.

Compensation

$43-$48 per hour equivalent, paid as salary based on established FTE, experience, and qualifications.

• 0.8 FTE / 32 hours per week: approximately $71,552-$79,872 annually

• 1.0 FTE / 40 hours per week: approximately $89,440-$99,840 annually

Mileage reimbursement is provided for eligible business travel.
