Job summary
Hospice of the West is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our nursing team in Mesa. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their journey.
The role includes Monday–Friday 8 am–5 pm with occasional on-call rotation, collaboration with the Clinical Director, and a focus on compassionate, clinically excellent care within a supportive team.
Qualifications
- Active RN license in good standing.
- Minimum 1 year medical-surgical/acute care experience; hospice experience a plus.
- Computer proficiency and EMR experience required.
- CPR certification may be required by state.
- Valid driver's license and reliable transportation.
Responsibilities
- Empower Through Engagement: respond to clinical referral information with care and expertise.
- Collaborative Care Champion: assess eligibility and suitability for hospice services.
- Nurturing Direct Care: deliver hands-on hospice nursing care and coordinate services.

- Educator and Advocate: educate patients and families for personalized care plans.
- Call of Compassion: share in on-call duties to be there when it matters.
